Support Coordination
Every participant's situation is different, so our support coordination is built around you. We work with people living with a wide range of disabilities, including psychosocial disability, physical disability, autism and neurological disability, among others.
Our experienced support coordinators and specialist support coordinators walk beside you through every stage of your NDIS journey. We help you understand your plan, find and connect with the right providers, and make the most of your funding, always with your growth, wellbeing and goals at the centre. The aim is simple: helping you use your plan to build the life that matters to you.
Three levels of support coordination
The NDIS can fund support coordination at three levels, depending on your needs:
- Support connection: help to understand your plan, link in with community and mainstream services, and grow your confidence to manage things yourself.
- Coordination of supports: hands-on help to organise and combine your supports so you can live more independently, keep up relationships and take part in your community.
- Specialist support coordination: a higher level of support for more complex situations, keeping your services steady and connected through challenging times.
- Putting your plan into action
- Finding and connecting providers
- Solving problems and barriers
- Preparing for plan reviews
Psychosocial Recovery Coaching
We empower NDIS participants living with psychosocial disability to take back control of their lives and set out on a recovery journey that truly means something to them.
A psychosocial recovery coach is an NDIS-funded worker who supports people whose disability relates to mental health. Your coach gets to know you and the people important to you, then works with you to:
- create and regularly review a recovery plan based on your strengths, hopes and goals
- build skills, confidence, resilience and decision-making through one-on-one coaching
- find, plan and organise the supports you need, and keep them working well together
- connect with mental health, health, housing, education, employment and community services outside the NDIS
- stay engaged and supported through times when you need extra help
- understand and get the most from the NDIS
Recovery coaching works alongside your clinical team, such as your GP, psychologist or psychiatrist. It does not replace treatment. You can choose a coach with lived experience of mental health recovery, or one with learned experience through training and work in the field.
Recovery coaching is a Capacity Building support. It is usually funded through the Support Coordination or Improved Daily Living budget in your plan. Recovery coaching and support coordination generally aren't both funded in the same plan, so we'll talk through which one suits you best.
